Are appointments confidential?
All information you provide is confidential and cannot be disclosed without your written permission. With a signed written release of information, I can collaborate with other treatment providers or individuals in your support system. There are circumstances in which I am mandated by law to break confidentiality, which are explained thoroughly in the consent paperwork.
What is your cancellation policy?
Appointments missed without notice or cancelled less than 24 hours in advance will be charged the full session fee. Exceptions can be made on a case-by-case basis for accident or emergency. I am also unable to hold appointment slots for more than two consecutively missed sessions.
What are your rates?
Please see the Services page for a detailed description of rates. A limited number of reduced rate slots are available each week. Please inquire regarding the availability of reduced rates.
Do you take insurance?
I do not accept insurance but can provide a superbill detailing dates of service, diagnosis, and type of service provided. It is your responsibility to determine what insurance benefits you receive and can obtain for reimbursement. If you are planning to submit the superbills I provide to insurance, please call your insurance company prior to your first appointment, and request further information regarding your out-of-network benefits.
How do I make a payment?
I accept all major credit cards as forms of payment. While I do not accept insurance plans, I can accept HSA debit cards. Payments are due at the time of service.
